A very specific sequence of events need to happen for a blood clot to form. The process is called the clotting cascade, and every step has to be successful for the clot to stop bleeding at the site of an injury. The importance of blood clotting is clear. What is factor Xa?

Xarelto and Factor Xa
Xarelto is an FDA approved medication that stops factor Xa from playing its part in the clotting cascade. When an individual takes this medication, the fibrin mesh is not formed so bleeding continues unchecked. Some individuals need to take this medication because they are at increased risk of complications from blood clots due to other illnesses. For example, those suffering from atrial fibrillation of the heart are more likely to suffer from a stroke because small blood clots can form in the heart from the disturbed rhythm. These clots then travel to the brain and block blood flow and oxygenation of vital brain cells.
The problem with Xarelto is there is no way to stop its effect once a dose has been taken. It has to wear off, and the drug lasts up to 24 hours. This can lead to massive hemorrhage, and if the bleed is in the brain, permanent brain damage and death.
Insufficient Warning
Many times this risk has not been adequately communicated to patients. The manufacturer of Xarelto, Janssen pharmaceuticals, has already settled with the Department of Justice for not adequately disclosing risks of other medications it has on the market.
